The Doctor's Choice Home Health is seeking immediate hire for an LVN Case Manager to join our growing team. The LVN will monitor the plan of care to reflect admission database, on-going findings, age-appropriate care, cultural specific needs, and appropriate acuity. We have to offices you can work from - either Los Angeles 90048 or Woodland Hills 91367
LVN Case Manager Responsibilities:
Leads effective development and completion of discharge plans including discharge barriers and patient/family education.Discusses w/ patient, family/significant others about the plan of care.Monitors the patient's progress based on their care plan.Revises plan on an ongoing basis based on patient condition and evaluation of progress in coordination with the clinical team.Answers patient questions about medication purpose, side effects, and administration instructions. LVN Case Manager QualificationsGraduate of an accredited practical nurse or vocational nursing program.One to Two year minimum clinical case management experience. Community health/hospice or medical/surgical experience is preferred.Currently licensed as an LPN/LVN in the State.Complies with accepted professional standards and practice.Demonstrates good verbal and written communication, and organization skills.There is also a requirement to be on-call for this position. It is done on weekends and works as a rotation 1-2x a month. A specific weekend would be designated to you, duties include answering calls on Saturday or Sunday 8 AM- 5 PM (virtually). On call rate discussed during interview.
