Come join our amazing team at Home Health and Hospice! We are seeking LPNs to join our fast-paced interdisciplinary team providing skilled nursing services to clients in their home. The Adult Home Care team delivers care to patients in the community, providing a wide range of skilled nursing care. We are a comprehensive team comprised of RNs, Physical Therapists, LPNs, LNAs, SLPs, Social Workers and administrative support, all collaborating together to support patients to remain in living in the setting of their choice, their home.
The Community Health Nurse, LPN will be responsible for the delivery of basic nursing care to patients in a home care settings, under minimal clinical supervision, as directed by a Registered Nurse, and in accordance with an established nursing plan of care.
At times travel may be required outside of assigned territory but a general service area is assigned and part of typical travel. Accredited School of Nursing LPN Program
Two or more years of directly related home/public health, and/or hospice experience
Current LPN licensure or compact licensure recognized by the State of Vermont required
Previous home care desired.
Must have the ability to push, pull and lift from the floor, up to a total of 50 pounds and be able to move with the weight, as to organization’s standards
Manual finger dexterity-will need dexterity to perform duties and use equipment
Visual acuity-will need sufficient eyesight to drive a car, use office equipment and perform duties
Auditory acuity- will need to assist individuals in person and by telephone
