We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.+ **Facility:** BayCare HomeCare+ **Location:** Bradenton, FL+ **Status:** **PRN**+ **Shift Hours:** 8:00 AM - 5:00 PM+ **Weekend Work:** Occasional+ **On Call:** Yes+ **Home visits in assigned service area:** **Manatee County**The **Per Visit Home Health LPN** provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. **Responsibilities include:**+ Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse+ Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse+ Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status+ Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.**Required Certifications and Licensures:**+ BLS (Basic Life Support)+ LPN (Registered Nurse) License+ Driver's License State of Florida (Florida driver's license is required within 30 days of hire)**Education:**+ Required Associate's or Diploma Nursing**Experience:**+ Required 1 year Licensed Practical Nurse+ **Preferred 1 year Home Care**Why join BayCare HomeCare?
