How You'll Make a Difference (Essential Functions)
The Resource Manager position is responsible for providing leadership, guidance, and direct supervision to the Resource team members on a day-to-day basis. This position ensures hospice services are patient and family centered and consistent with the philosophy, goals, standards, regulations, and policies of Agrace. This position maintains accountability for quality outcomes, financial performance, and the overall growth and development of the team. This position is responsible for allocating staff resources based on patient acuity and level of need within the clinician's scope of practice, ensuring quality and continuity of care. The Resource manager promotes a positive and innovative culture through coaching.
Key Responsibilities
- Coordination and Delivery of Home Care Hospice Services by Resource Staff
- Ensure delivery of high quality, consistent, efficient, and cost-effective Agrace services by Resource staff.
- Manage appropriate staffing level and allocate services based on census and patient/caregiver needs.
- Patient assignments to teams/care groups ensuring consistency of staff, when applicable, and maintaining established care group matrices.
- Audit documentation to find trends, implement continuous improvement processes and ensure compliance with state and federal regulation.
- Proactively work with team to address patient, caregiver, and safety concerns (patient or staff).
- Provide leadership support and guidance to 24/7 operations in on-call capacity as needed.
- Quality and Service
- Monitor team performance ensuring continuous quality improvement and high levels of patient, family and partner satisfaction.
- Educate direct care staff in understanding, embracing, and applying responsibility in creating exceptional quality and service outcomes.
- Participate in Quality Assurance Performance Improvement (QAPI) workgroups and assist with QAPI initiatives.
- Address quality and/or safety concerns including those entered into agency Incident Management System.
- Financial Performance
- Review staff productivity and create action-plan for addressing issues and needs.
- Ensure team follows established requisition for expenditures, following formularies, protocols, and standards both for visit assignments and related vs. unrelated products and services.
- Management of Staff and Employee Engagement
- Participate in recruitment process and make hiring decisions for new team members.
- Supervise applicable homecare disciplines including:
- Monitor supervisory visits ensuring they are completed, and successful completion of skill assessments/simulations.
- Coach staff to work within established discipline guidelines and maintain boundaries.
- Ensure all team members complete all required competencies timely and successfully.
- Facilitate performance management communication for Resource team. (i.e. warnings, performance improvement plans, etc.)
- Assess educational needs of team and facilitate their development and knowledge into practice.
- Work with preceptors to evaluate readiness of new staff to work independently.
- Promote culture of learning, recognize concerns, resolve fears, and motivate the team for engagement.
- Other Duties
- Participate in internal work groups, meetings and committees relative to position responsibilities.
- Escalate need for staff education and/or resources to address gaps in care or knowledge deficits.
- Ensure compliance with Agrace policies and protocols, state, federal, and other regulations.
- Associate Degree in Nursing, required. Bachelor's degree, preferred
- Licensure: Current, unrestricted Wisconsin RN license
- 5 years clinical leadership and/or a healthcare administrative role, preferred; hospice environment, preferred
- Ability to work regular weekend rotation
- Valid driver's license, auto insurance, and access to a reliable vehicle
- Ability to communicate financial/business/technical/clinical information to all levels within and outside the organization including group presentation skills
- Experience in project management, strategic planning and execution, budgeting, and controlling costs
- Intermediate Microsoft Office Suite skills
- Experience using electronic health record software
- Ability to use proper body mechanics and standard precautions
