- Found in 2002, the FQHC is a team setting that provides compassionate care regardless of income status
- This health center has 5 locations, 25 providers, and 400 team members with solid tenure with many team members, 15 yrs to 20 years
Benefits and Features:

Your Role with the Company: - Full time, Monday-Friday 8am-5pm
- APP will work at one location - open locations: Fitchburg, Leominster, and South Gardner
- Team based mindset with collaborative environment
- FNP / PA will work in a primary care setting seeing 18-24 patients per day
Background Profile: - Board Certified Family Nurse Practitioner
- 2+ years of experience as a Family Nurse Practitioner / FNP or Physician Assistant / PA
- Active MA License
