Key Responsibilities
Conduct comprehensive psychosocial assessments within 5 days of admission and as needed. Participate actively in the interdisciplinary group (IDG) to help develop and revise individualized plans of care. Provide emotional and psychological support to patients and families throughout the end-of-life journey. Assist families with advance care planning, funeral planning, and navigating difficult decisions. Coordinate access to community resources such as financial assistance, housing, food, transportation, and caregiver support. Provide crisis intervention and grief support before and after death, including participation in the bereavement program. Educate patients and families about hospice philosophy, services, and their rights. Maintain timely and accurate documentation in the electronic health record (EHR). Collaborate with chaplains, nurses, physicians, and other team members to ensure quality, coordinated care. Participate in quality improvement initiatives and in-service training as required. Qualifications
Bachelor's Degree in Social Work (BSW) required; Master's Degree in Social Work (MSW) preferred. Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the State of [Insert State]. Minimum of one (1) year of experience in a healthcare or hospice setting preferred. Strong communication, problem-solving, and interpersonal skills. Ability to work independently and as part of a team. Valid driver's license and reliable transportation for home visits. Working Conditions
Hybrid role including patient homes, skilled nursing facilities, and office settings. May involve occasional evening or weekend visits as needed. Requires ability to handle emotional situations and provide support in times of grief and loss. Benefits
