Position Overview:
Greenlife Healthcare Staffing is seeking a compassionate Licensed Practical Nurse (LPN) to join a five-star rated Assisted Living Facility in Medford, NY. - Education: Diploma/Certificate from an accredited LPN program.
- Licensure: Active New York State LPN License.
- Experience: Minimum 6 months of clinical experience (assisted living or geriatric care is a plus).
- Technical Skills: Proficiency in care planning, EHR documentation, and preventive/rehabilitative nursing procedures.
- Soft Skills: Strong communication, empathy, attention to detail, and teamwork.
Key Responsibilities:
The Adult Care Facility Nurse, LPN will perform tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse.
- Calls the home care coordinator, RN or supervisor with verbal report of initial findings.
- In conjunction with the family and physician and/or authorized practitioner, the American Supervisor adheres to the Plan of Care based on the RN's assessment.
- Provide nursing care as outlined in the Plan of Treatment to include the following: Nursing services, treatments, and preventive procedures, including those requiring substantial specialized skill and ordered by the physician or authorized practitioner.
- Initiates preventive and rehabilitative nursing procedures as appropriate for the patient's care and safety.
- Evaluates the effectiveness of his/her nursing service to the individual and family.
- Observes signs and symptoms and changes in the patient's physical or emotional condition and consults with the physician or authorized practitioner concerning alteration of the plan of treatment in consultation with the supervisor, as necessary.
- Provides and completes nutritional assessments under direction of Director of Nutrition.
- Reports all patient changes and information to the Primary RN, Supervisor or Case Manager of home care on a timely basis.
- Submits timely records of visits and vouchers based on each visit.
