JOB SUMMARY:
Provides clinical expertise, assists in clinical development, working collaboratively within an interdisciplinary care team, ensuring standards of care and providing consultation and expertise in the delivery of patient care. Activities include consultation as requested, assistance in case conferencing, clinical supervision, provision of patient care and clinical over site for Hope Hospice and assisting the medical director and associate medical director in conducting patient visits including face to face encounters.
This is a Pro Rata position or as needed, with no set hours nor benefits eligibility.
QUALIFICATIONS / EXPERIENCE:
Current Texas License as an RN. Current Authorization to Practice as an APN in the state of Texas. This may include Interim Approval to Practice or Provisional Authorization to Practice or Full Authorization to Practice according to the Texas Board of Nurse Examiners Advance Practice Nurse Regulations Rule 221. Limited Prescriptive Authority By Texas Board of Nurse Examiners and Texas Board of Medical Examiners. Master's degree in nursing or related field. Certification of completion of an Advanced Practice Nurse Program. Current National Certification as a Nurse Practitioner. This certification may be pending successful completion of the national exam. Preferred specialty area(s) include one or more of the following: Adult Nurse Practitioner, Gerontological Nurse Practitioner, Family Nurse Practitioner, Acute Care Practitioner, or Psychiatric/Mental Health Practitioner. Current National Certification in Hospice and Palliative Care (CHPN), preferred, but not required. Certification in Education for Physicians in End of Life Care (EPEC) preferred. Hospice or Palliative care experience preferred, but not required Strong communication and assessment skills. Reliable transportation, proof of automobile insurance. CLINICAL:
A Hospice physician or Hospice nurse practitioner must have a face-to-face encounter with each Hospice patient whose total stay across all hospices is anticipated to reach the third (3'd) benefit period, and must continue to visit that patient prior to every recertification thereafter, to gather clinical findings to assist Hospice in determining the patient's continued eligibility for hospice services ("Patient Visits"). Upon the request and authorization of Hospice, Physician shall travel to the residences of Hospice patients, if necessary, and perform Patient Visits and, if appropriate, complete the recertification and prepare the physician narrative, in accordance with Hospice's policies, procedures and applicable laws and regulations ("Services"). Physician shall perform at least one (1) Patient Visit per month. Services shall be provided in a safe and effective manner and, as applicable, in accordance with the patient's plan of care. Documentation. Physician shall document Services in accordance with professional standards, Hospice's policies and procedures and applicable laws and regulations. Physician shall provide designated Hospice personnel with documentation of each Patient Visit within eight (8) hours of conducting the Patient Visit. Physician shall also complete a timesheet, on a form provided by Hospice, and submit the timesheet along with mileage documentation to Hospice by the 5"i day of each month that document the Patient Visits performed by Physician for the prior month. Works collaboratively with the Director of Patient Care Services and other team members to ensure smooth day-to-day operations within the teams, ensure the care provided to patients meets agency and regulatory standards, quality patient care, and effective and efficient resources utilization. Ensures adherence to Hospice Conditions of Participation. Ensures excellent patient care services Assists Hospice Medical Director and Associate Medical Director in conducting hospice patient home visits, palliative care consultation visits and face to face encounter visits.

WORKING CONDITIONS:
Driving to and visiting in patient homes and nursing homes that may include climbing stairs and variable circumstances. May be required to lift, carry, push and pull greater than 50 pounds, repeatedly. Potential for exposure to blood and body fluids and infectious material. Also function in normal office environment; moderate amount of standing, walking, stooping, sitting, reaching.
