Centers Health Care . Primary Vascular Care is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to perform on-site clinical assessments for residents of client facilities. The position reports to the Lead Nurse Practitioner. Key Responsibilities: Perform on-site screening assessments to determine patient treatment plans using comprehensive assessment forms. Conduct independent history and physical examinations for complex, acute, critical, and chronic patients. Order and review diagnostic and therapeutic tests appropriate for the patient's needs. Assist in providing care in compliance with facility, state, and federal regulations. Monitor intervention effectiveness and maintain relationships with facility contacts. Educate families, clients, and prospective clients about services offered. Support physician relations and complete reports timely. Qualifications: Bachelor's Degree required. Graduation from an accredited Nurse Practitioner or Physician Assistant Program required. Current NP License required. Basic Life Support (BLS) certification required; ACLS preferred. Infection Control Certification required. Board Certified or Eligible required. Vaccinations and tests: PPD/TB, Hep B, MMR, Varicella required; COVID and Flu vaccines preferred.
