Annandale Village is a one-of-a-kind nonprofit community in Suwanee, GA, where we proudly serve adults with developmental disability and acquired brain injuries. We are a true community where our residents, known as Villagers, are supported in living with dignity, purpose, and independence.
We're the only nonprofit in the southeast offering a full continuum of care for the people we serve. Our 55-acre campus includes: Two assisted living buildings serving 33 Villagers Eight Personal Care Homes (independent, semi-independent, and cottages) serving up to 100 Villagers A 32-bed skilled nursing facility-the only one in Georgia dedicated solely to adults with developmental disabilities A Medicaid waiver day program with approximately 33 participants A community-based independent living program, All In, currently serving 26 individuals Annandale Village is synonymous with home. Position Purpose
Through the use of recognized counseling methods, assures the best quality of life for the Villagers at the Personal Care Homes & Assisted Living; serves as an advocate for the Villagers; communicates effectively with families, Annandale staff and other agencies to meet the needs of the villagers; provides documentation of villager activities goals and achievements.
Required Qualifications
Minimum of a bachelor's degree in social work or related human services field with 2 or more years' experience in a human services setting, preferably in ID/DD/MH.
A master's degree in a related field preferred.
Knowledge of Applied Behavioral Analysis techniques and data collection preferred.
Must be at least 21 years of age, have a valid Class C Georgia Driver's License, proof of vehicle insurance, and meet company policy regarding MVR requirements.
Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.
Must undergo and pass a drug screening and screening for Tuberculosis.

Major Duties and Responsibilities
Supports Villagers to live as independently as possible in the Personal Care Homes and the Assisted Living
Supports Villagers regarding issues related to personal and family adjustments, finances, employment, food, clothing, housing and physical or mental health concerns in order to determine the nature and degree of the problem and helps the Villager to come to a resolution.
Secures information such as medical, psychological, and social factors which contribute to a Villager's situation, evaluates these, then assist the Villager to resolve the situation within their capacity.
Reports abuse or suspicion of abuse as required by Annandale policy and licensing agencies.
Helps Villagers modify attitudes and patterns of behavior through a team approach and assistance from a Behavioral Analyst.
Initiates small group outings or individual outings as needed to ensure that all Villagers have opportunities to participate in their community.
Compiles records, oversees Villager's mail and Villager accounts, monitors Villager off campus outings, and maintains the Villager's care plan in ALIS, communicates with families per their preference.
Schedules and facilitates Care Plan meetings for each Villager on caseload and incorporates information into the electronic medical records system. Facilitates support team communication and meetings; including following up on concerns expressed by team members and coordination care and support.
Responsible for completing documentation within the established timelines in the electronic medical records system, including but not limited to documentation of family notification in the Incident Reporting system.
Responsible for ensuring that all required documentation is in the Villager files and the electronic medical records system from intake to discharge.
Conducts skills classes based on the Villager's Assessment of Functional Living Skills (AFLS) and individual needs.
Works with P&S Team, Director of Villager Support Services and Behavioral Analyst to develop goals for villagers from their AFLS assessments; encourages Villagers to participate in the PC curriculum designed to help them meet goals and follows up to ensure success.
Assists Health Services department by reporting Villager health concerns.
Completes quarterly care plan updates to ensure that goals and action items have been completed to ensure the quality of service provided to the Villager.
Participate in the utilization of Interns according to their specific area of focus and/or assignment.
Assists, as necessary, with Villager ADLs for Villagers.
Participates in social events and activities as needed, such as parties, outings, etc.
Monitors Villagers in cafeteria at mealtime as needed.
Attends all required and assigned trainings and meetings.
Leads or assists as needed in a crisis intervention situation.
Performs other duties as needed that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care needs of the Villagers.
Job performance will be assessed at least annually using the Annandale Village performance evaluation process.

Employee is required to lift up to 25 pounds and/or move up to 75 pounds (with assistance). Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Employee may be required to transport (drive) and assist Villagers in and out of vehicles. Employee is required to perform CPR/First Aid.
