Compass Healthcare Consulting & Placement is conducting a search for a Hybrid Remote and Field RN Care Manager for a Managed Care Plan servicing New York. The RN Care Manager position works as a Partial Remote Case Manager, working primarily from home, with up to 2 days of Field Case Management visits per week, in designated area. Bilingual Chinese Registered Nurse needed for Manhattan, Queens or Brooklyn, NY visit areas, Bilingual Spanish Registered Nurse needed for the Bronx, NY.
Qualified candidates will be able to work from their homes and will make an occasional Field visit, approx. 2 days per week in field. Qualified candidates will have prior RN Case Management experience within Managed Care, Home Care or Hospital Case Management, bilingual Spanish or bilingual Chinese language ability required. Qualified candidates will have a NYS RN License or NYS NP, and prior RN MLTC Case Management experience.
Patient visits area available in Manhattan, Queens, Brooklyn or the Bronx, NY. Bilingual ability required.
Hybrid Remote & Field - RN Care Manager Responsibilities: Reporting to the Supervisor of Care Management, this position will provide telephonic and in-person case management to our members including medical, social, psychological, physical and spiritual needs by developing, implementing and monitoring the member's care plan in conjunction with the PCP, caregivers and other team members in order to help meet the member's needs.managed care nurse, managed care rn, nursing, rn, nurse, care management Qualifications: NYS RN license required CCM preferred At least 2+ years of RN Case Management experience in managed care MLTC Ability to make patient visits up to 2 days per week, ability to work remote from home the remaining days Reliable transportation Covid19-Vaccination Bilingual Chinese or Bilingual Spanish Prior experience in an interdisciplinary service delivery environment preferred. Working knowledge of Medicare and Medicaid regulations Will consider RN's with MLTC or CHHA Home Health Care Management experience
