Chesnut Hill of East Longmeadow, a Skilled Nursing Facility in East Longmeadow, MA, is currently seeking applicants for an experienced MDS Coordinator. Requirements: Graduate of an accredited school of nursing with current LPN/RN licensure in Massachusetts One year of Long-term care experience Experience with Medicare/Medicaid reimbursement, MDS completion, clinical resources utilization, and/or case management is highly desirable Responsibilities: Completing accurate assessments, MDS & care plans as assigned Initiating care plans and supporting activities as assigned Creating and distributing monthly care plan calendars in a timely fashion Maintaining and updating care plans and assessments Monitoring and auditing clinical records, ensuring accuracy & timeliness Must be in compliance with MDS / Medicare protocols and policies.
