Job Description
PURPOSE OF POSITION:
Serves as the facility's SUD Women's Health Ambassador as part of the Addiction Consult team and the patient's ambulatory OB care team, conferring and consulting with ED/IP/OP clinical service leads/providers to provide a multidisciplinary approach to SUD patient care

SUMMARY OF DUTIES AND RESPONSIBILITIES: • As a member of the multidisciplinary NYC H+H Addiction Consult team under the clinical direction of the Associate Medical Director, provides timely and complete screening, brief intervention, and psychosocial assessment to patients with or at risk for a substance use disorder (SUD) or of complications of substance use located within an Acute Care setting [Emergency Department (ED), Inpatient (IP), Birthing Unit, Bridge, or Ambulatory clinic]; prioritizing perinatal patients in these areas • Serves as the facility's SUD Women's Health Ambassador as part of the Addiction Consult team and the patient's ambulatory OB care team, conferring and consulting with ED/IP/OP clinical service leads/providers to provide a multidisciplinary approach to SUD patient care • Assesses and identifies the patient's level of care, making recommendations or referrals, referring to a higher level of care as needed (inpatient detox or rehab) • Completes intakes for H+H MSOP and OTP as necessary, coordinating follow-up services • Conducts patient treatment/discharge/aftercare planning as it relates to SUD in collaboration with care team • Provides direct care individual, group counseling, and outreach services; making referrals to community-based providers as well as setting up patient transportation as appropriate • Maintains a small caseload of perinatal Ambulatory clinic patients at with or at risk for SUD, providing bridging counseling services until long-term SUD treatment services are established • Escorts patients for SUD placement interviews/discharge when appropriate and indicated by supervisor/designee • Escorts patients for SUD placement interviews/discharge when appropriate and indicated by supervisor/designee • Provides opioid overdose education and harm reduction resources to patients and their family members or support networks • Interviews patients' relatives, support network, facility staff, and members of the community regarding patients' ability to function in the community as appropriate • Participates in interdisciplinary Performance Improvement (PI) and Quality Improvement (QI) projects • Attends Addiction Consult team case conferences, trainings, and meetings such as within Acute Care and Ambulatory clinic service areas where perinatal patients are treated as well as rounds as it relates to SUD; participates in work of selected staff committees • Orients and educates members of other professional disciplines on substance use counseling concepts and functions • Works with care team as indicated to create plans of safe care and abide by regulations in reporting data on maternal substance inclusive of the Child Abuse and Prevention and Treatment Act (CAPTA) guidance • Attends conferences on Women's Health, inclusive of the Child Abuse and Prevention and Treatment Act (CAPTA) • Keeps electronic records, prepares reports and related paperwork, ensuring patient confidentiality • Reports suspected cases of child abuse or neglect to the New York Central Registry, working with appropriate Child Protection resources • Oversees complex cases requiring an advanced degree of knowledge and expertise • May coordinate and preliminarily review the work of one more Addiction Counselor Level I clinician assigned to the ACS team • May supervise Addiction Counselor students in their field placement, as well as volunteers and other auxiliary personnel as needed • May orient Addiction Counselor Level I clinicians, providing on-the-job clinical/workflow training • May act as a liaison between supervisory and line staff, conveying feedback with regard to workflow and program implementation • May provide coverage in any of the facility's other SUD service areas as needed • Other duties as assigned by supervisor/designee
Minimum Qualifications
Any individual working in a NYS Office of Addiction Services and Supports (OASAS)-Certified Treatment Program or newly-hired into the System in the title of Addiction Counselor in a NYC Health + Hospitals health care setting must have proof of completion of OASAS "Supporting Recovery with Medications for Addiction Treatment (MAT)" training. For Assignment to Level II 1. Valid CASAC - Level 1 Certificate issued by OASAS; and one (1) year of full-time, progressively responsible experience performing SUD assessment, treatment planning, individual and group counseling, case and care management and other functions related to SUD prevention and treatment in a SUD treatment program; or 2. Is otherwise considered a QHP, as described in "2" of Qualification Requirements for Level I above, and has one (1) year of full-time, progressively responsible experience performing SUD assessment, treatment planning, individual and group counseling, case and care management and other functions related to SUD prevention and treatment in a SUD treatment program; or 3. Satisfactory equivalent of education, training, and experience. However, all must have a CASAC Level 1 Certificate issued by OASAS or meet the definition of a QHP as described in "2" above.
