Responsibilities
Job Summary / Purpose
The Specialty Clinic RN navigates patients through the continuum of cardiac care providing individualized and comprehensive care to reduce barriers. The RN will provide education, interpretation and support for cardiac patients. The nurse serves as the primary point of contact coordinating and collaborating with providers and clinical staff for care planning and services across the continuum of care. The RN will effectively utilize resources and teams to identify and eliminate barriers to care in order to achieve and develop a long term relationship and optimal clinical outcome for the patient.
Essential Key Job Responsibilities
Demonstrates the knowledge, skill and coordination to provide nursing care and guidance to the cardiac patient from first contact and throughout follow-up, serving as an essential link between patients and all other care providers.
Systematically and continuously performs the functions of assessing, planning, implementing and evaluating the care according to ACOG and CHI SaintJoseph Health standards of practice.
Facilitates the coordination of patient care services to assure excellence in patient care and patient flow by: a) following the patient through the care continuum/experience, eliminating operational (i.e. scheduling, test results, etc.) barriers to service; b) working closely with other health care disciplines to ensure timely appointments, results reporting, financial need referrals, communication, patient care and follow-up.
Interfaces with other healthcare teams for appropriate referrals/services such as the social worker, pastoral care, dietician, community services, financial counselor, pharmacy coordination, home care, smoking cessation, etc.
Initiates and documents patient teaching including family and significant others based on assessment of needs, including responding to patient requests for information regarding the disease process, expected side effects of treatment and community resources. Documents the above appropriately as well as communicating teaching completed to the patients other health care providers as appropriate.
Instruct, track and communicate procedures for cardiologists with the patient and hospital coordinators to ensure quality patient care.
Continually assesses the patient's progress and educational needs, making adjustments to care based on the assessment; consults with primary physician or designee and other members of the care delivery team as needed; refer to appropriate physicians, NPs or programs when needed.
Coordinates patient admissions ordered by physicians and instruct patients prior to date of arrival
Follow patients for Amiodarone surveillance and ensure appropriate testing and labs are completed in a timely manner
Prioritize patient calls and triage their needs with guidance from our physicians
Assist certified medical assistants and device technicians within the EP department with patient care
Other duties as assigned by management

ADN or Prefer Bachelors of Science in nursing
Required Licensure and Certifications
KY RN licensure
Required Minimum Knowledge, Skills and Abilities
Minimum of 5 years of experience in inpatient or outpatient Cardiac setting, or related experience required
PREFERRED Qualifications
Experience/knowledge in community health setting or services desirable.EP
Pay Range
$25.18 - $36.52 /hour
