Primary Duties and Responsibilities:
The Senior Practice Consultant will be accountable for working with Tandigm's network practices on an in-depth, consultative basis to identify and implement strategies to improve various aspects of accurate coding including: Comprehensive Visits, medical documentation, provider/staff coding acumen, and EHR configuration/utilization in the pursuit of various initiatives in population health.
In addition to working with network practices, the Senior Practice Consultant will lend their deep subject matter expertise to the ongoing operations and QA activities for ACE, including incentives, MSSP programs, and collaboration with other functional areas such as quality. From time to time the Senior Practice Consultant will assist with business initiatives to improve the programs, processes, and analytics used in ACE.
The ideal candidate will have experience and exceptional judgement interacting with clinical leaders and practice management personnel regarding complex practice management strategies around Accurate Coding and Documentation, QI initiatives, and improving practice management capabilities. Essential Functions Closely collaborates with Tandigm's AD/Liaison personnel assigned to each practice, ensuring a continuity of messaging and coordination of initiatives. Serves as a trusted and strategic partner for physician practices within Tandigm's provider network. Maintains expert-level understanding of the CMS-HCC, RxHCC, and HHS-HCC Risk Models, along with strong knowledge of the CMS Stars program. Maintains expert level understanding of the regulatory landscape as it pertains to risk accuracy, including CMS regulations, guidance from industry associations such as AHIMA, ACDIS, and AAPC. Develops and maintains strong knowledge of EHR platforms most commonly used throughout the Tandigm network, with a focus on reporting, QI capabilities, and workflow design. Effectively analyzes each practice's unique approaches to operational and clinical workflows, developing insights as to how these effect the practice's performance with Tandigm's incentive program. Works with clinical and operational leadership at individual and corporate practices to provide consulting services, assessing current systems and processes and identifying recommendations for the implementation of clinical and operational strategies to improve risk accuracy and overall practice performance with the Tandigm incentive plan. Consistently exhibits behavior and communication skills that demonstrate Tandigm Health's commitment to superior customer service, including quality, care and concern with each and every internal and external customer. Uses, protects, and discloses Tandigm's patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards and protects Tandigm assets from unauthorized access, disclosure, modification, destruction or interference. Adheres to and responsible to abide by all information security policies, processes, and/or activities. Reports security events or potential events or other security risks to the organization to management. Performs additional duties as assigned. Required Qualifications: Education
Greater than 10 years of relevant professional experience; formal clinical training preferred
Licenses & Certifications
Valid U.S. State Driver's License as well as proof of liability and property damage insurance on any vehicle used for business purposes. Experience
Minimum
5 or more years clinical or management experience in a healthcare or population health environment Knowledge of CMS-HCC, RxHCC, and HHS-HCC risk models, along with strong ICD-10 acumen Strong technical skills including computer and software skills such as Microsoft Word, Excel, PowerPoint, and Outlook Experience with multiple EHR platforms Strong presentation capabilities Knowledge of health plan and regulatory requirements Preferred
Accomplished in public speaking Practical experience with Lean Six Sigma or similar process improvement methodologies Knowledge, Skills, Abilities
