Up to $5000 Sign-On Bonus Available Department Details: We are seeking a compassionate and qualified BSW or MSW to join our hospice team and provide support in the Luverne and Worthington areas. Compensation: BSW:

$26.50 - $38.50 Job Summary: As a Hospice Social Worker, you will: Provide supportive services/counseling related to healthcare and home care programs. Work collaboratively with an interdisciplinary team to address the social, emotional, and economic needs of patients and their families. Facilitate crisis intervention and assist families in navigating complex medical situations. Coordinate care and resources from various social, health, and human service agencies. Respond to issues of suspected abuse, neglect, or violence per ethical and legal standards. Conduct care plan development with a focus on maximizing patients’ quality of life. Exhibit professionalism in care conferences and maintain effective communication with all stakeholders. Qualifications: Bachelor's Degree in Social Work from an accredited program is required. Experience in healthcare or mental health settings is preferred. Basic Life Support (BLS) certification to be completed within six weeks of employment, if applicable. Must have a valid license in good standing in the state of practice. For BSW: Licensed Social Worker (LSW); for MSW: Licensed Graduate Social Worker (LGSW) or Licensed Independent Social Worker (LICSW).
