Role Description
This is a part time, per diem hybrid role for an Aging Life Care Manager at Visionary Care Consultants. The role involves day-to-day tasks related to care management, elder care, social services, and medicine. This role requires travel within San Diego county.
Position Summary:
The Aging Life Care Manager is responsible for the coordination and delivery of services to clients, ensuring individualized care plans are developed, implemented, and monitored effectively. This role involves advocacy, resource navigation, and ongoing evaluation to support positive client outcomes.
Key Responsibilities: • Assess client needs through interviews, records review, and collaboration with professionals. • Develop, implement, and monitor individualized service plans. • Maintain accurate and up-to-date documentation in accordance with agency standards and regulatory requirements. • Coordinate referrals to appropriate services and resources. • Act as a liaison between clients, families, service providers, and community resources. • Monitor client progress and adjust plans as needed to ensure goals are being met. • Ensure services are provided in a culturally competent, respectful, and client-centered manner. • Attend team meetings, training, and case conferences as required.
Qualifications: • Bachelor’s degree in Social Work, Gerontology, Psychology, Nursing or a related field (Master’s preferred). • 5+ years of experience in case management or elder care services. • Strong problem solving, communication, and interpersonal skills. • Proficiency in client management systems and use of technology • Knowledge of local resources, social services systems, and community programs.
