Overview
Performs comprehensive wound assessment for all patients.
Orders medications, treatments, monitoring for efficacy and for possible side-effects and laboratory and other diagnostic tests, following up with results.
Communicates with collaborating physicians within and outside AHS when appropriate.
Consults with patients and families regardign goals of care. Follows patients through various transitions of care, to and from hospitals, facilities, and home, coordinating with in-hospital staff, facility staff and home care agencies to ensure continuity of care.
Provides comprehensive documentation in an electronic medical record regarding disease states, medications, treatments, and the results of interventions.
Answers phone calls from pharmacies, caregivers, patients, and facility staff.
Provide education and mentoring to student nurses, medical students and residents during their clinical wound rotations.
Ongoing professional education to keep up to date with wound clinical practice guidelines.
Maintaining state licensure and CE requirements necessary for licensure.
Participates in performance improvement activities/ quality measures as related to area of specializing. Investigates, evaluates and recommends supplies and capital equipment. Supports preparation for Joint commission, DOH, and other regulatory visits. supports implementation of policies, guidelines, and recommendations of the regulatory bodies. Evaluates quality improvement data and formulates evidence-based recommendations to improve quality care and nursing practice.
Advocates for patients, family members, and nursing personnel. Supports policy and service that advocates for patient rights and optimal environments of care. Plans with staff to identify interventions within the plan of care that reflects current scientific knowledge; practice and promote continuity of care. Performs evidence- based interventions consistent with patients/ family needs. Implements pharmacological and non-pharmacological interventions for patients within the framework of state licensure and hospital privileges.
Manages and assists to meet health education needs of patients and families. Coordinates and/ or provides education in areas of specialty to meet the clinical and professional development needs of nursing staff.
Required:
Master's degree in Nursing.
Current NJ state Nurse Practitioner license.
Preferred:
Certification by the ANC/ AANP for Adult Nurse Practitioner.
Wound and Ostomy certification.
