About the Role:
We are seeking a dedicated VRS and HSS Case Manager to join our team at Kesma Flame Lily, LLC in Minneapolis, MN. This role offers the opportunity to make a meaningful impact in the lives of our clients and contribute to the growth of our company.
Responsibilities:
- Provide case management services to individuals utilizing (Independent Living Skills) and Housing Sustainability Services (HSS)
- Assess client needs and develop individualized service plans
- Coordinate with healthcare providers, social services, and other relevant agencies
- Advocate for clients to ensure access to necessary resources and support
- Document client interactions and progress in case management software
Requirements:
- Bachelor's degree in social work, counseling, or related field
- Experience working with individuals in housing sustainability, and independent living skills
- Knowledge of ARMHS and HSS programs and services
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
