Assigned county Territory is Area Area #2 Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont and Area #8 Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington TWO Options - Guaranteed Hours or Non Guaranteed hours. Current CA OT License BLS Drivers License Auto insurance Auto Registration TB (annual) MMR Varicella HepB TDap (10 years) Flu (annual) Drug screen Physical Schedule Information
If Guarantee, the following shall apply: Productivity: At least 27 points per week. 36 hours Guarantee. Traveler must complete 30 points in the week. If Non-Guarantee, the following shall apply: Weekend coverage may be required. To secure a full caseload, Traveler may be required to go outside the service area occasionally. Additional Details
Your Agency's contract to your traveler MUST mirror the confirmation page. If your agency does not comply with this standard, your agency will be at risk of being removed from this client's vendor set. No exceptions! Start Date Must be Identical. End Date Must be Identical. All Requested Time Off (RTO) must be disclosed at the time of submission.
