Your Role as a PT As a valued member of our team, you will: Perform personalized in-home therapy sessions, developing tailored treatment plans that deliver real results. Guide patients in improving strength, balance, and mobility, reducing fall risks, and enhancing independence. Educate patients and families on safe daily living activities, such as using assistive devices and preventing falls. Document treatment progress, patient status, and outcomes using the WellSky/Kinnser EMR system. Collaborate with physicians and case managers to deliver seamless care. Who We're Looking For We're seeking passionate, self-motivated professionals who: Hold a valid California PT license. Have AHA CPR certification. Are reliable, organized, and personable. Have a minimum of one year of experience (Home Health experience is a plus). Have reliable transportation, a valid driver's license, and auto insurance.
