Certification Details BLS Massachusetts Physical Therapist License Job Details Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease, or other causes. Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs. Job Requirements Physical Therapist Professional license in Massachusetts is required. 1 Year Home Care experience required. Schedule Information Weekend rotations are once every 6 weeks. Symmetrical headshot, shoulders up, look straight/no tilted head. No eyeglasses. MVR check to be run by background check company within 45 days of start. Required for all home care positions. Primary Source Verification of professional license and/or certification, as required for the position. Within 30 days of start date and updated upon expiration. N95 - within 365 days to start date and updated annually. Within 30 days of start. Required by MA DPH for all employees. A completed primary COVID-19 vaccination series that is approved by the FDA or the World Health Organization (WHO) OR an up-to-date COVID vaccine if a primary series was never received or completed is required. Documentation of two measles vaccines, two mumps vaccine, and one rubella (German measles) vaccines or two MMR vaccines. Must have documentation of TB skin test screening within 3 months of start date and updated if >6 month break.
