What We Offer: $24.28-$29.74/hour DOE with the ability to go higher for highly experienced candidates

Generous PTO, 8 paid holidays, and much more! What You'll Do: Build program-specific caseload by seeking referrals from providers and partners
Conduct intake screenings to assess clients' needs and determine appropriate case management services
Conduct in-depth client interviews to determine current status and barriers
Refer clients to appropriate agencies/services based on their needs
Assist clients with the self-referral process
Regularly meet with clients to monitor service plan implementation
Create, update, and maintain client service plans in accordance with guidelines
Establish and maintain relationships with service providers/agencies
Participate in meetings, including interdisciplinary team case reviews
Perform clinical responsibilities in alignment with regulatory requirements Qualifications: Bachelor's Degree in a social service field such as Social Work, Behavioral Sciences, Psychology, or Child Development OR
Associate's Degree with two years of relevant experience OR high school/GED with four years of relevant experience
Bilingual (English/Spanish) preferred at level 10 for bilingual differential pay
Valid Driver's License and proof of automobile liability insurance coverage required
Effective verbal, written, and listening communication skills
Knowledge of medical terminology and local social and health services
Ability to develop effective relationships with social service providers and partners
