Corewell Health Urgent Care is looking for an Advanced Practice Provider (APP) to join their practice at the Grand Haven Urgent Care as well as the Zeeland location caring for patients across the generational spectrum. The urgent care group is 60+ APPs strong plus a handful of physicians. As a provider in these spaces you will be credentialed at multiple locations and will typically work at 2-3.Family Nurse Practitioners or Physician Assistants required as we care for patients of all ages.Team-based care is supplemented with medical assistants, radiology technicians, practice managers and access to consult all specialties for the Urgent Care Space.Urgent Care Hours will be 8am to 8pmEssential Functions+ Functions in the role of physician assistant to coordinate comprehensive care delivery. Writes orders/prescriptions medications, durable medical equipment, physical therapy, laboratory, radiographic studies and consultations, coordinating diagnostic evaluations, follow-up exams, laboratory tests, referrals to home care, social services, and other community agencies, etc., in order to ensure appropriate continuity of care and coordination of services.+ Obtains histories and performs physical exams.+ Assesses the patients/significant other, health care team, school and other community educational needs and develops, implements, evaluates and revises teaching strategies/educational programs related to area of specialty.+ Acts as a patient advocate, crossing multidisciplinary lines to meet patient needs. Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Spectrum Health. Acts as a consult in area of expertise.+ Maintains current knowledge and skills in areas pertinent to the delivery of urgent care medicine.+ Able to perform all waived laboratory tests provided on premises in urgent care environment. Performs and assists with designated procedures.+ Documents in the medical record and dictates information as appropriate in a timely manner. Completes all necessary documentation to provide accurate and efficient billing for services.+ Provides input into the strategic planning, implementation and evaluation of service/program.Qualifications :Nurse Practitioner:+ Required Master's Degree Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a masters, post-masters or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission+ 3 years of relevant experience current, relevant clinical experience+ Previous experience functioning in a collaborative role as a Nurse Practitioner+ LIC-Nurse Practitioner (NP) - State of Michigan+ CRT-Basic Life Support (BLS)+ CRT-Neonatal Resuscitation Program (NRP)+ CRT-Pediatric Adv Life Support (PALS)Physician Assistant:+ Required Master's Degree Graduate of an accredited Physician Assistant educational program+ LIC-Physician Assistant - State of Michigan+ CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants+ CRT-Basic Life Support (BLS)+ CRT-Pediatric Adv Life Support (PALS )+ CRT-Neonatal Resuscitation Program (NRP)Corewell Health:People are at the heart of everything we do, and the inspiration for our legacy of outstanding outcomes, innovation, strong community partnerships, philanthropy, and transparency.
