Job Title: Physical Therapist Job Specialty: Physical Therapy in Home Health Job Duration: 13 weeks Shift: 0800-2000 Guaranteed Hours: 40 hours per week Experience: 1 year of home health experience required License: State license for Physical Therapy required Certifications: Basic Life Support for Healthcare Provider certification from the American Heart Association required Must-Have: - Experience with HCHB EMR - Experience with geriatric/pediatric therapy, chronic disease management, and maintenance - OASIS-E experience - Ability to work within the geographic area of Concord, Kannapolis, Salisbury - Proficiency in English (Speak, Read, Write)Job Description: - Evaluate, plan, direct, and administer programs of Physical Therapy to restore function and prevent disability. - Conduct Physical Therapy programs to help patients enhance physical and functional abilities. - Evaluate patients and select assessment techniques to establish appropriate care plans. - Gather and interpret data from medical records, physician reports, and patient and family feedback. - Establish functional goals with patients and provide evidence-based interventions to achieve them. - Document evaluations, tests, progress notes, daily treatment notes, and discharge notes. - Communicate patient evaluations and progress to physicians, referral sources, and clinical staff. - Coordinate patient schedules, treatment programs, and discharge planning. - Supervise PTAs and rehab tech/aides as required for licensure. - Perform physical requirements such as lifting, lateral transfer of patients, and demonstrating exercises. - Use personal protective equipment as necessitated by potential exposure to hazardous conditions. - Work in a fast-paced clinical environment, predominately indoors with occasional outdoor activities. - Adhere to the dress code of grey scrubs.
