Full Time 11p-7a with every other weekendPart Time 7a-3p (2-3 days per week)The primary purpose of this job is to provide person centered direct care that addresses the dignity and individual preferences of each person; providing optimum physical comfort, hygiene, mobility and general well-being for each resident as directed by the individualized care plan. The CNA works under the instruction and observation of a licensed nurse. In many ways, Certified Nurse Assistants are the front line hands and eyes of any health care team. They observe resident's physical, mental and emotional conditions and report any changes to the charge nurse. Job Description:Recognize and respond to the needs of residents. Assist the resident with "activities of daily living", including bathing, dressing feeding, toileting, lifting, turning, moving, positioning, and transporting residents into and out of bed, using chairs, bath, lifts or other assistive devices and appliances per the care plan. Other duties are assigned by supervisor. CNAs are subject to be called back in emergency situations or disasters such evacuations related to tornados.JOB DUTIES AND RESPONSIBILITIES:The duties of the Certified Nursing Assistant include, but are not limited to:Stays familiar with the person centered care plan for each resident assigned and incorporates changes to the care plan into the patient care routine as directed by supervisorFollows federal and state standards of care within scope of responsibilityOffers daily assistance or performs tasks necessary for hygiene, hydration, repositioning and mobility and comfort of each resident. This includes bathing, shampooing, dressing, undressing, feeding or assisting with meal set up, assisting with ambulation, assisting with toileting or changing briefs if resident is unable to use a toilet, skin care and perineal care for skin integrity, assistance oral care, care of dentures, eyeglasses and hearing aidsAdapts caregiving routine to accommodate therapy, activities, resident outings and appointments outside the facilityOffers meals, therapeutic snacks and liquids as ordered and upon request to residentsDocuments daily care, including food, therapeutic snacks as ordered and liquid consumption per facility protocols and plan of care as required. Documents bowel and bladder activityWeighs residents using upright, bed or chair scale and records accurate weights per care plan or supervisor instructionsRecords intake and output per protocolReports any concerns, complaints, reports of abuse, neglect or misappropriation to supervisor immediatelyReports any unusual resident behaviors, signs of depression or changes from usual routine or mood to supervisor immediatelyReports any physical symptoms, changes in bowel or urine changes/sudden decline in awareness or abilities to supervisor immediatelyDisposes of soiled linen and clothing per facility protocols and maintains a safe and clean environment in resident rooms and bathroomsUses universal precautions and follows special instructions for infection control and isolationFor safety of both employee and resident, uses gait belt, shower chairs, lifts, scales, electric controls on beds and equipment responsibly, appropriately and correctly for tasks as trained. Reports any malfunctioning, fraying or broken equipment immediately to supervisorAnswers call lights promptlyObserves privacy and dignity of residents and does not share resident medical or psychosocial information with other residents, visitors or any unauthorized personsDevelops and maintains a good working rapport with other departments with the facultyConsistently demonstrates professionalism and confidentialityParticipates in quality assurance program and attends appropriate QA committee meetings as directed by supervisorServes on other facility committees as requestedAttends all mandatory in service including annual fire, OSHA and CDC programMaintains current CPR/BLS certificationPractices in adherence to national standards for electronic health care transactions and code sets, unique health identifiers, and security, including passwords and user ID codes, in compliance with the Privacy Act and HIPAA. Notifies Administrator, or Director of Nursing in the absence of the Administrator, of any known violationsJob Requirements:Must have GED or high school diploma. Must have training from an approved CNA training program; Must have CNA certification in Tennessee, in good standing; prefer one or more years of experience in SNF/NF setting. Must pass criminal background check. Must not be listed on National Sex Offenders Registry or any abuse registryMust be able to read, write and speak EnglishMust have the ability to make independent decisions within scope of practice when circumstances warrant such actionsMust meet the general health requirements set forth by the policies and procedure of Stone River Manor. Must be able to push, pull, move, or lift a minimum of 25 pounds to a minimum of five (5) feet and able to push, pull, move and carry such weight a minimum distance of fifty (50) feet. Must be physically able to evacuate residents in the event of evacuation. Must be able to cope with mental and emotional stress of the position. Must possess hearing and sight, with or without use of prosthetics that enables these senses to function adequate to fully meet the requirements of the job, which include taking vital signs of temperature, blood pressure, pulse and oxygen saturation. Must be able to work with people of varying levels of understanding, mood or coping abilities, varying cultures and varying sensory awarenessMust be able to use electronic charting system for documentationMust be familiar with and practice in adherence to national standards for electronic health care transactions and code sets, unique health identifiers, and security, including passwords and user ID codes, in compliance with the Privacy Rule and HIPAAWe do notdiscriminate based on a person's national origin , race, color, religion, disability, or sex.
