At Pride Health, our personalized approach is the foundation of honest and open relationships with our network of healthcare professionals, creating an unparalleled environment of trust and loyalty.Job Title: Wound, Ostomy, and Continence NurseJob Summary:Under the direction of the Wound and Ostomy Manager, the Wound, Ostomy, and Continence Nurse will provide safe, competent, quality care based on nursing theory and research to a designated group of patients.Duties and Responsibilities:Conduct health assessments, develop care plans, and recommend treatments focused on wound, ostomy, and continence care.Collaborate with the healthcare team, making referrals as needed, and participate in quality improvement activities.Educate patients, families, and staff on preventing complications related to ostomies, wounds, or continence issues.Assess and document patient responses to treatments and evaluate the quality of care provided by clinical staff.70% of the role will be in office, nurse might be required to travel to patients home (MA, BKLYN, BX &QU) to complete wound care consults.Skills and Experience:Minimum two (2) years of experience in a Certified Home Health Agency or post-acute care facility.Minimum of 2 years of Public/Community Health and 2 years medical/surgical experience required. Previous experience in PI, i.e., knowledge of PI applications, tools and techniques and knowledge of the development, coordination and facilitation of PI programs.Previous experience using various computer software applications: i.e., Word, Excel, Access, etc.Certifications and Licensure:Current New York State Registered Nurse LicenseCertification as a Wound Care Nurse or Wound Ostomy Care NursePosition Details:Location: New York, NY 10004Position Type: 12 WeeksPay Rate: $75.00 - $80.00 Per HourShift and Time: 9A - 5PJoin us in making a difference in healthcare!
