Street Medicine Clinician (NP/PA)The Street Medicine Clinician role focuses on delivering compassionate and high-quality medical care to unhoused individuals, directly in the environments where they reside-such as parks, encampments, and under bridges. The goal is to reduce barriers to care and provide medical and social services on their terms. The Street Medicine Clinician will work collaboratively with a multidisciplinary team, connecting patients with needed care, services, and resources. This role is on-site in Midtown Sacramento, CA (95811). Schedule Requirement: minimum 32 hours per week.Essential FunctionsProvide direct primary and acute care to unhoused patients in non-traditional settings, such as streets, shelters, parks, and encampments.Perform thorough medical evaluations, diagnoses, and treatments, including management of chronic conditions, mental health support, wound care, and substance use treatment.Facilitate access to necessary health services, including medication management, specialty referrals, diagnostic testing, and preventive care.Provide care within a trauma-informed, patient-centered framework, prioritizing dignity and respect in all interactions.Accurately document patient encounters, assessments, and treatment plans in the electronic medical record (EMR) while ensuring timely documentation for billing purposes.Collaborate with social workers, case managers, and community outreach workers to address social determinants of health, connect patients to housing, and improve overall health outcomes.Educate patients on health management, disease prevention, harm reduction strategies, and available resources.Understand and maintain compliance with Quality Improvement Program indicators, medical clinic policies, and applicable state and local laws and regulations.Participate in regular team meetings, street rounds, and case reviews, offering insights on patient care, program development, and outreach strategies.Assist in developing and refining clinical protocols and procedures tailored to street medicine practices.Understand and comply with universal precautions, particularly in non-clinical settings, and ensure team members adhere to safety standards.Promptly communicate operational or compliance concerns to Medical Leadership and implement corrective strategies as necessary.Additional DutiesParticipate in workgroups, teams, task forces, and committees to support ongoing program and clinic functions.Foster communication and collaboration within OCH and external agencies to enhance service delivery to patients.Responsible for monitoring and reporting potential child, adult, elder, and dependent adult abuse in accordance with applicable laws (Penal Code, Section 11166, Section 15630, and Section 1160).Perform other related duties as assigned by Medical Leadership.Travel ResponsibilitiesPrimarily local with occasional travel to community sites and partner organizationsTraveling to encampments, shelters, and other sites to meet the needs of the unhoused individuals.Candidate should possess reliable transportation, valid driver's license, and car insurance.Minimum RequirementsFully licensed as a Nurse Practitioner or Physician's Assistant in the state of California. 1-2 experience.Valid DEA certificate and current Basic Life Support (BLS) certification.Demonstrated ability to work with underserved and marginalized populations, with a focus on the unhoused.Commitment to providing equitable, patient-centered care in non-traditional and sometimes challenging environments.Preferred BackgroundExperience in an FQHC or community health center.Experience in family medicine, with a focus on underserved populations.Familiarity with substance use treatment, behavioral health care, and street outreach programs.One Community Health serves a widely diverse patient population with a rich blend of races, ethnicities, cultures, ages, religions, disabilities, sexual orientations, gender identities, and socioeconomic backgrounds.
