Summary:

The Integrated Care Coordinator (ICC) is a patient-facing care coordinator for dental services. This role provides patients with direction, health education, insurance coverage information, and support in navigating/accessing integrated health services within the Village Health Center (VHC). This role works closely with the Dental Director, Patient Service Representatives (registration), Outreach & Enrollment Workers (benefits assistance), Referrals Specialists, and other VHC team members to assure patients receive timely, comprehensive care. The ICC is dental-centric, ideally having prior work experience rendering dental services (as a DA/RDA) and possessing understanding of how oral health is connected to the rest of the body. The ICC will provide chairside dental assisting as part of their duties in addition to coordinating care of dental patients with other VHC providers. They serve as a source of support for dental patients who require “warm hand-offs” between care services to assure high-quality care and patient stabilization. They also reach out to patients being seen in other departments to establish dental care. The ICC outreaches and engages all VHC patients to reduce “gaps in care,” with a particular focus on senior health care. They also facilitate medical consults of patients for the dental team when needed. This role may deploy into other areas of Father Joe’s Villages (FJV) or the local community to provide support to patients. The ICC is self-motivated and takes initiative to approach patient and providers to secure services that aid in the goal of integrating all VHC services to all patients with priority focus for seniors (ages 60+).Essential Functions:Provides care management and health care navigation services within a Federally Qualified Health Center (Village Health Center; VHC) serving individuals and/or families experiencing homelessness.Uses strong customer service skills to perform outreach and engagement in-person and via phone, assessment/evaluation of needs, provides education, meets and/or distributes materials to patients, coordinates and schedules services, and/or performs warm hand-offs to needed services.Serves as a representative of Father Joe’s Villages (FJV) to the Senior Oral Health Partnership (SOHP) and will attend meetings, coordinate activities, and report to the SOHP in collaboration with the Dental Director. Flexible to meet with community partners and participate in meetings, as needed, to meet funder requirements.Possesses strong organization, prioritization, and time management skills as well as the ability to work autonomously.Ability to review communications and respond on multiple platforms, checking each multiple times per session.Verifies insurance eligibility for services, as appropriate, and facilitates meetings for patients to apply for insurance with eligibility and enrollment specialists.Participates as a member of a multidisciplinary team. Effectively communicates and collaborates with patients, other health professionals, clinical staff, and colleagues.Works to create positive working relationships with VHC and FJV team members as well as community resource providers that benefit VHC patients. Creates and maintains a contact list with phone, fax and email information.Participates in monthly outreach events in or around the VHC.Participates in meetings, presentations, workgroups and/or learning collaboratives, as assigned and attends the weekly clinical coordination meeting.Facilitates outreach and referrals to other organizations in an attempt to secure services unavailable at Father Joe’s Villages Dental Clinic but recommended by the dental providers.Searches the patient database to identify seniors who may need navigation of services and help to facilitate such.Works with health care providers to facilitate services to patients from dental to other VHC departments and vice versa.Serves as the “go-to” person for dental-related matters for scheduling and coordinating care between the dental clinic and the other departments within the organization.Works as a chairside dental assistant, as assigned.Maintains records of interactions and outcomes of efforts to coordinate care.On time completion of assigned trainings and policies.Performs other duties as assigned.Additional Essential Qualities:The ICC is responsible for scheduling, organizing and managing multiple aspects of a patient’s healthcare maintenance and treatment.The ICC employs strong organizational skills necessary to systematize a care plan for several patients at once, while coordinating with diverse healthcare workers who hold varied schedules.The ICC has professional verbal and written communication skills that expedite and facilitate patient understanding of health needs while maintaining a high level of respect and confidentiality for the patient.Qualifications:High School Diploma or GED. Bachelor’s degree preferred in related field.Minimum 2 year experience in a healthcare setting. Equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the essential job duties.Bilingual (Spanish/English) written and verbal, required.Completion of a Dental Assistant training program preferred but will train on the job for the right candidate.Experience with electronic health record systems. Basic user of MS Office, and other essential computer skills.Current American Heart Association healthcare provider CPR (BLS) certification.Experience supporting individuals with disabilities, age-related conditions, mental illnesses, or challenging behaviors highly preferred.Knowledge of Medi-Cal, Medicare, Managed Care Plans, and insurance guidelines preferred.Participate in an annual Tuberculosis screening and/or other screenings, as assigned.Reasonable accommodations may be granted where appropriate.
