Position Summary

Responsible for performing aspects of the Patient Flow Management process ensuring that transfers, patient placement and related activities occur in a timely, safe and accurate fashion. This includes providing clinical assessment for patients being placed through the Center for Patient Flow Management.

This RN position supports our system-wide Transfer Referral Center, located at the UH Management Services Center in Shaker Heights. The hours are 36 hours per week on straight nights (7p-7:30a).

Essential FunctionsCoordination of incoming patient transfer requests with receiving facility, physicians, and necessary transport resourcesDemonstrates technical skills in care delivery that result in safe practice and positive care outcomesDemonstrates clear communication and effective collaboration to optimize patient care and clinical operationsDisplays leadership that influences optimal clinical and operational outcomesDelivering exceptional customer service while facilitating communication across multiple disciplinesPossess and obtain working knowledge of external and inter-facility protocols and standards of practiceWork with all Center for Patient Flow Management (CPFM) staff in a team orientated, collaborative approachResponsible for promoting a Best Place to Work Culture and positive and professional work environmentRadio communication and coordination with multiple agencies, including Air Medical programs and UH Critical Care TransportDispatch of UH Critical Care Transport ambulancesOther duties as assignedRequired For All Jobs

Education

(BSN) Bachelor's Degree in Nursing . PreferredWork Experience

5+ years of acute care hospital in-patient nursing Required andICU or ED experience PreferredKnowledge, Skills, & Abilities

Excellent interpersonal and communication skills both written and oral RequiredStrong organizational and multi-tasking skills RequiredDemonstrates a professional demeanor when interacting with patients and customers RequiredStrong computer skills including all MS platforms RequiredExcellent ability to perform triage in order to determine appropriate facility, bed type, service, etc RequiredLicenses and Certifications
