at Wilmington Primary Care located at 501 West 14th Street in Wilmington, DE .
You will be joining a team of physicians, advanced practice clinicians, Registered Nurses, medical assistants, and office assistants to support this very busy residency practice.
Work Schedule:Monday through Friday, 8:00am to 4:30pm and 8:30am-5:00pm rotation.No weekends or holidays.Compensation / Benefits:Full Medical, Dental, Vision, Life Insurance, etc.Two retirement planning offerings, including 403(b) with company contributions.Generous paid time off with annual roll-over and opportunities to cash out12 week paid parental leaveTuition assistanceIncredible Work/Life benefits including annual membership to care.com, access to backup care services for dependents through Care@Work, retirement planning services, financial coaching, fitness and wellness reimbursement, and great discounts through several vendors for hotels, rental cars, theme parks, shows, sporting events, movie tickets, insurance and much more!Job duties & responsibilities:Performs Annual Wellness Visits, Advance Care Planning and Transitional Care Management calls and disease management.Triages walk-in patients and ensures appropriate dispositionTelephone triageAnswers and responds to telephone messages under the direction of the providerAssists providers in examinations or proceduresFunctions as a liaison between patients, physicians and staff to provide accurate communicationAssures adequate coverage within unit when neededSupports the work of LPN and/or MAAssists with staff education and evaluation of clinical competencies for new and existing employees (MA & LPN)Assists with orientation of new personnel to ensure that they are properly trained to support patient and provider.Performs chart audits or has designee assist with chart auditsCase Management and interdisciplinary collaborationChronic disease management under provider directionPerforms nurse appointments for patient education and medication/vaccine administrationIn office medication administration and wound careFull spectrum family medicine practice - meaning prenatal care as wellHelp to support Substance Use Disorder Programs in Family Medicine office and Mobile Van. Includes coordination of care and patient outreach.Education Requirements:A graduate of a state approved or National League of Nursing accredited School of Nursing.RN license or eligible for licensure in the state Delaware.Two years’ experience as a RN, physician office experience preferred.BSN preferred; ADN considered with agreement of completion of BSN in 3 years.Special Requirements:Current RN licensure in the State of DelawareBLS certification requiredTo be considered, please apply online and attach a current resume.#L1-CS1ChristianaCare Job ID #JR74594.
