ChristianaCare is seeking a Registered Nurse (RN) Clinic for a nursing job in West Grove, Pennsylvania.Job Description & RequirementsSpecialty: ClinicDiscipline: RNDuration: OngoingEmployment Type: StaffChristianaCare is currently seeking a casual Registered Nurse to support our Primary Care Practice located in West Grove, PA one day per week.As part of a team of 6 RNs, this role provides phone triage support for 5 other primary care practices within the regional pool.Work ScheduleOne eight hour day per week (Monday)No holidays, weekends, or late hoursPrincipal Duties and ResponsibilitiesDemonstrates knowledge & skills necessary to provide care appropriate to pediatric, adolescent, adult and geriatric patients virtually or on-site within the practice, including knowledge of growth development, the ability to obtain and interpret information to identify patient needs and provide care neededParticipates in a relationship-based care approach to care deliveryPerforms Nurse Visits such as Annual Wellness Visits, Advance Care Planning and Transitional Care Management calls and disease managementPerforms point of care tests, EKG’s and other tests as directed by cliniciansTriages walk-in patients and ensures appropriate dispositionAnswers and responds to telephone messages under the direction of the providerAssists providers in examinations or proceduresFunctions as a liaison between patients, physicians and staff to provide accurate communicationCollaborates with team members to provide a smooth, organized and efficient flow of patientsResponds to patient crises situationsAssures adequate coverage within unit when neededProvides patient education and wellness counselingPerforms medication historySupports the work of LPN and/or MAAssists with staff education and evaluation of clinical competencies for new and existing employees (MA & LPN)Assists with orientation of new personnel to ensure that they are properly trained to support patient and provider.Performs chart audits or has designee assist with chart auditsRandomly shadows clinical support staff to ensure appropriate care is providedSupports the site by acting as the liaison for JCAHO, POCT, Tracer monitor for Safe Practice Behavior Monitoring (SPBM) tools & JCAHO TracersAdministration of medications, monitoring of vital signs, and performing routine treatments and procedures prescribed by the physician when needed.Keeps current with policies and procedures and participates in quality improvement activities.Participates in special projectsPerforms assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.Performs other related duties as required.Education and Experience RequirementsA graduate of a state approved or National League of Nursing accredited School of NursingRN license or eligible for licensure in the state Delaware and PennsylvaniaTwo years’ experience as a RN, physician office experience preferredBSN preferred; ADN considered with agreement of completion of BSN in 3 yearsSpecial RequirementsCurrent RN licensure in the state in which the work is performedBLS certification requiredQualified candidates should apply online and attach a current resume for consideration.#L1-CS1ChristianaCare Job ID #JR73602.
