Coverage Location (Facility Name, City, State): Ascension St. Mary's Hospital, Saginaw, MichiganPractice Setting (Free-Standing Pathology Lab/Hospital-Based Pathology Lab): Hospital basedReason for Coverage: Gap in coverage, as other pathologist is leaving for another opportunityDates of Coverage: Starting the end of June, ongoingSchedule (Shifts and Hours): Mon through Fri., 8:00am - 5:00pmCall Schedule: Weekly (alternate week), start at Friday 5:00 pm, finish at next Friday 8:00amCall Ratio: every other weekWhat Type(s) of Certification Does Your Laboratory Hold (CLIA, CAP, etc.)?CLIAApprox.
