The Corner Health Center is Michigan's first, largest and most comprehensive adolescent and young adult health center. The Corner serves youth and young adults ages 12 through 25 (and their children) and has over 40 years of experience providing care to individuals with complex needs. The Corner provides high quality, judgment-free, affordable health and wellness care and education.

Job Summary

The School Wellness Program (SWP) Nurse is responsible for overseeing students who have consented to receiving services. In this role, the SWP Nurse provides direct care nursing, promotes health education, and guarantees compliance with state health regulations and The Corner Health Center (TCHC) policies and standing orders. The SWP Nurse works closely with TCHC staff, school staff, families, and community healthcare providers to support student health and safety.

Essential Duties and Responsibilities

Nursing Care:
  • Promotion of healthy behaviors, education, nutrition, physical activity, and mental health awareness among students and staff.
  • Assessment of minor injury and illness, administration of medication according to standing orders and school policies.
  • Provision of Coordination of care and chronic disease management with the school and primary care/specialty provider.
  • Collects and maintains consent documentation from parents/guardians and students for care provision and coordination.
  • Maintains accurate health records and documentation for students, ensuring confidentiality and compliance within state and federal regulations and TCHC policies.
  • Coordinates Telehealth visits as needed with consent of parent/guardian and student.
  • Ensures immunization records and health screenings are up to date in accordance with Michigan Department of Health and Human Services (MDHHS) guidelines. Conduct health assessments and screenings as needed. Review Immunization status per MCIR and provide immunizations per standing orders as needed.
  • Performs point of care and send-out lab testing, follows results, and communicates results to parents/patient and PCP as indicated.
  • Maintains emergency response plans and ensure staff training in CPR, First Aid, and Emergency Protocols.
  • Provides education for staff, colleagues, patients and families
  • Provides and documents individualized health education for patients and families
  • Provides confidential care in accordance with state and federal guidelines and Corner Health policy.
  • Upholds the policies set forth by The Corner Health Center.
  • Maintains inventory of medications, supplies, and vaccines
  • Provides Medicaid outreach services to eligible youth and families adhering to the Child and Adolescent Health Program criteria and the law.
Youth Advocacy:
  • Advocates for Adolescent Health initiatives
  • Provides nonjudgemental care, support, health education and promotion
  • Ensures confidentiality of care, empowering patients to make healthy choices
  • Provides age appropriate health and lifestyle education
  • Provides overview of services at TCHC and coordinate refer for services and programming as appropriate
Quality Assurance, Data Collection and Reporting:
  • Upholds mission, goals, and policies of The Corner Health Center.
  • Participates in Quality Assessment and Improvement initiatives
  • Maintains Quality Assurance protocols for clinic and lab procedures and results reporting
  • Maintains compliance with regulatory standards regarding practice, environment, and documentation.
  • Utilizes TCHC's EMR for documentation of consent, patient records, and results tracking
  • With TCHC staff, collects and reports on data related to patient care, outcomes, and trends to identify opportunities for intervention and education.
  • Monitors patient schedule to ensure regular followup and tracking.
  • Maintains database and creates reports. Participates in CAHC reporting.
Collaboration:
  • Works closely with school staff, WIHI SWP Program Coordinator and Social Worker, TCHC staff to support student health and academic success.
  • Communicates concerns and recommendations effectively with parents/guardians and healthcare providers.
  • Collaborates with local health departments, community organizations, TCHC and other healthcare providers to coordinate services and resources for students.
  • Provides professional development and/or training to school staff relevant to current needs.
  • Communicates with TCHC clinic manager and Medical Director regarding patients at risk, and meets quarterly with the Medical Director for chart review.
Qualifications:
  • Registered Nurse (RN) with a current license to practice in the state of Michigan.
  • Bachelor's degree in nursing (BSN) preferred.
  • Experience working with child/adolescent populations
  • CPR certification and proficiency in first aid.
  • Knowledge of Michigan's health education standards and regulations.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Previous experience working in a school setting required.
  • Demonstrated interest and experience in health education. Comfort leading education and training sessions for individuals and groups.
  • Compliance with TCHC employee guidelines, including vaccination against Influenza, COVID, and screening for Tuberculosis.
Work Environment:
  • School hours, Monday to Friday.
  • Works in a patient care setting with possible exposure to communicable diseases
  • The work environment includes the SWP clinic at WIHI, school facilities such as classrooms, offices, as well as The Corner Health Center.
