Job Description:

40 Hour Monday-Friday

Director of Social Services

The

Social Services

Director is responsible for implementing and documenting a plan of care related to the resident’s social/emotional needs in the electronic medical record. The

Social Services

Director collaborates with the nursing, MDS, and rehabilitation departments to facilitate discharge plans for residents. The

Social Services

Director informs and educates the staff, residents, families, and responsible parties of the resident’s personal and property rights, as well as rights related to health services. The

Social Services

Director maintains updated resources for residents and families regarding community resources and services, as well as service agencies and organizations. This role maintains a record of all reported concerns, assists in resolving them, and ensures appropriate follow-up and documentation. The

Social Services

Director is easily accessible to residents, families, and staff, providing timely referrals to appropriate agencies for assistance (e.g., conservatorship application, Medicaid application).

Qualifications:

Bachelor's or Master's Degree from an accredited program in Social Work or related field. BSW Required.

Previous experience in a long-term care facility and familiarity with current Requirements of Participation regulations is preferred.

Knowledgeable of admissions and discharge procedures in conjunction with all current laws, as well as familiarity with residents’ rights and documentation requirements.

Attention to detail, good follow-through skills, and ability to prioritize multiple tasks.

Innovative and able to find creative ways to serve the variety of resident needs while maintaining a customer focus.

Must have a vehicle and a valid driver’s license.

Demonstrates cooperation in assisting others and ability to work as a team.

Patience, tact, enthusiasm, and a positive attitude toward residents and their support networks.

Ability to remain calm in stressful situations.

Ability to communicate with residents, staff, and family members in a professional manner.
