The Advanced Practice Provider Float works to support the mission and vision of Neighborhood Healthcare by providing excellent patient-centered care and services within a dynamic team environment. This role will provide direct healthcare services to patients under the direction and responsibility of Physicians in three main categories: direct patient care, team support, and clinical administration.

RESPONSIBILITIES

DIRECT PATIENT CARE

Performs comprehensive physical examinations on patients, including review of patient medical data, such as health historyOrders, administers, and interprets test results for diagnostic tests, such as x-ray, electrocardiogram, and laboratory testsPerforms therapeutic procedures such as injections, immunizations, suturing, and wound care, and managing infectionInstructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenanceImpacts patient experiences by demonstrating courteous and helpful behavior and a commitment to accuracyAssesses and provides age-appropriate care to patientsTEAM SUPPORT

Shares accountability for overall patient health outcomes, working in coordination with care teamsOperates to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholdersResponds promptly and with caring actions to patients and employeesMaintains professional working relationships with all levels of staff, clients, and the publicCooperates in accomplishing communicated department goals and objectiveCLINICAL ADMINISTRATION

Develops and implements patient management plans, records progress notes, and assists in provision of continuity of carePerforms in accordance with NHC practice protocols and under the guidance and supervision of the supervising MD(s), according to the terms of the Supervisory AgreementKeeps informed of organizational activities and promote mission and goalsFunctions at highest level according to credentials and competencyEDUCATION/EXPERIENCE

Master's degree from an accredited physician assistant or nurse practitioner program requiredValid CA Physician Assistant or Nurse Practitioner certification requiredValid CA DEA license requiredValid BLS certification from approved American Health Association provider required upon hireADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)

Excellent verbal and written communication skills, including superior composition, typing and proofreading skillsAbility to interpret a variety of instructions in written, oral, diagram, or schedule formKnowledgeable about and experience with providing direct care to patientsAbility to successfully manage multiple tasks simultaneouslyExcellent planning and organizational abilityAbility to work as part of a team as well as independentlyAbility to work with highly confidential information in a professional and ethical mannerPhysical Requirements

Pay range:

$147k to $199k

per year, depending on experience. (DOE)

Neighborhood Healthcare requires employees to be fully vaccinated (including a booster dose) against COVID-19 and demonstrate proof of vaccination/booster upon hire or receive approval for an exemption based on a medical reason, disability or religious belief. Proof of booster dose will be contingent upon booster eligibility.
