60% FTE, Days. Monday through Friday from 8:00 am to 5:00 pm. Hours may vary based on the operational needs of the department. You will work at 5249 East Terrace Drive in Madison, WI

As an Advanced Practice Provider, you'll play an integral role in ambulatory care team by providing high-quality, cost-effective care to patients and their families.

We are seeking a

Nurse Practitioner or Physician Assistant

to:Participate as a care team member in managing a panel of patients in partnership with our primary care physicians.Evaluate and treat acute and chronic health conditions and promote wellness in our outpatient primary care pediatric clinic.Provide counseling and education about common pediatric illnesses and promote wellness with patients and families.

Education :

Minimum -

If Nurse Practitioner :

Master's or Doctorate's degree from accredited Nursing Program

If Physician Assistant :

Bachelor's or Master's degree from accredited PA Program

Licenses and Certifications :

Minimum - State of Wisconsin NP or PA licensure

If Nurse Practitioner*:Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice ANDActive board certification as a NP in area of clinical practice. These may include the following: Pediatric Primary Care or Family Medicine.

If Physician Assistant*:Active PA license, including prescriptive authority, in state(s) of practice ANDActive PA board certification

All Nurse Practitioner and Physician Assistant applicants must have*: Active DEA license or ability to apply for such license prior to or upon hire.

*Active license or eligibility to obtain active licensure in state of practice. This may include licensing in Wisconsin and other state(s) of practice.
