Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Concordia Visiting Nurses is hiring for a full time Nurse Practitioner to cover our South Hills service area! This home health position is Monday through Friday,

no weekends, and no on calls!

DESCRIPTION

The Certified Registered Nurse Practitioner (CRNP) is a licensed health care provider who collaborates with the Clinical Team in implementing and evaluating the patient plan of care for appropriateness to each individual patient's needs to ensure the highest quality of healthcare is delivered. Care for the patient is given where the patient resides (i.e., patient's home, facility, hospital).

QUALIFICATIONS

Current CRNP license in the state of Pennsylvania2-3 years' experience in patient care requiredDemonstrates excellent clinical skillsCurrent CPR certificationSolid computer skills, including WordExcellent organizational skills with attention to detailOutstanding communication skills (both written and verbal)Knowledgeable of agency policies, both federal and state regulations and third-party payersDUTIES AND RESPONSIBILITIES

Clinical Skills

Provides skilled nursing care that includes activities such as teaching, treatments, (including IV therapy, venipuncture, complex wound care) and health promotion practicesExecutes preventive, rehabilitative and restorative nursing procedures appropriate for the patient's care and safetyAssesses signs and symptoms in conjunction with reporting reactions to treatments, including drugs, and changes in patient's physical, mental, or emotional condition to the physicianDevelops the plan of care collaboratively with the physician, patient, designated representative, and the agency with revisions made based on patient conditionInstructs and counsels the patient and the designated representative regarding the care requirements of the patient in the homeAssigns and instructs the Licensed Practical Nurse (LPN) or home health aide in accordance with the physician's plan of careSupervises and evaluates the care given by the home health aide to his/her primary patients every 14 daysSupervises and periodically evaluates the care given by the LPN to his/her primary patients to ensure oversightEvaluates and manages LPNs and home health aides who are providing services to his/her primary patientsIn collaboration with the primary clinician, provides what is necessary for the patient's need to obtain positive medical outcomes while managing appropriate resources.Obtains a medical history, particularly as it relates to the present condition, from the patient and/or designated representative at the start of care.Conducts a comprehensive physical examination and nursing assessment of the patient at each skilled visitEvaluates the patient and the home situation to determine the health teaching required at each skilled visitEvaluates the patient's condition and home situation to determine what assistance will be available from the designated representative in caring for the patientCoordinates total patient care serving as the primary nurse for designated patients and evaluating the effectiveness and the appropriateness of careInterprets nursing and other services of the agency to the patient and designated representative as part of planning for careServes as the primary agency liaison and coordinator of care when nursing and other services are providedPlans with the patient, designated representative, and other team members for the patient's discharge from serviceMeets with the interdisciplinary team routinely to ensure coordination of care and to update the plan of care as appropriateMeets with Clinical Supervisor or Case Manager weekly for an evaluation of patients under his/her careCommits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards, and Concordia Visiting Nurses policies and procedures; maintains confidentiality of patient and employee records and information; has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to the Executive Director and/or Compliance ManagerIs able to provide correct teaching on use of the home telemonitoring unit as well as basic troubleshooting to patients, designated representatives and other clinicians as needed.
