LPN Case ManagerHealthCare Support is actively seeking a LPN Case Manager to fill an opening with a Fortune 500 Company in Kissimmee, FL.Daily Responsibilities for LPN Case Manager: Discharge Planning Provide a point of communication between specialist. clinician and loved ones. Arrange referrals or recommend patient specialist where needed Evaluate the progress and recovery of the patients recovery after discharge and seen in the office. Ensuring that patient records are thoroughly and accurately updated.
