Travel Nurse RN - Home Health in King Of Prussia, PA

**Experience:** Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience

**Certifications:** CPR, BLS

**Must-Have:**- Proficiency in HomeCare HomeBase (HCHB) and used it within the last year- Proficiency in OASIS and used it within the last year- Reliable transportation, current driver’s license, and required liability insurance- Willingness to travel up to 75 miles from home clinic

**Job Description:**- Provide, coordinate, and direct home nursing care according to physician's orders based on agency policies and procedures- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses- Perform home visits with periodic office meetings- Manage an average of 5-8 patients per day- Handle various types of cases, including Start of Care, Wound Care, and Follow-up Visits- Complete documentation within 24 hours of patient appointments- Must be open to on-call rotations and willing to float to nearby clinics if necessary- Travel may involve longer drives between patient homes with mileage reimbursed at standard rates- Adhere to dress code and use electronic medical record systems as required
