**Experience:** Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience

**License:** Must have a valid nursing license in hand

**Certifications:** CPR, BLS

**Must-Have:**- Experience with HomeCare HomeBase (HCHB) EMR and proficiency in OASIS within the last year- Reliable transportation, a current driver's license, and required liability insurance- Willingness to travel up to 75 miles from the home clinic- Open to on-call rotations and weekend work as required

**Job Description:**

- Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.- Conduct home visits and participate in periodic office meetings.- Manage a caseload of 5-8 patients per day.- Complete documentation within 24 hours of patient appointments.- Achieve weekly productivity expectations of 30 points.- Be open to floating to nearby clinics if patient census is low.- Participate in weekly clinic meetings.- Maintain complete and timely medical records for each patient.- Efficiently plan travel routes to optimize time and minimize travel between patient homes.
