Job Title: Registered Nurse Job Specialty: Hospice Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not guaranteed Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on specialty License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase (HCHB) and Oasis; Recent specialty experience required Job Description: - Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures.- Responsible to the Clinical Manager.- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.- Conduct home visits and attend periodic office meetings.- Must have reliable transportation, a current driver's license, and agency-required liability insurance.- Manage a caseload of 5-10 patients per day, equating to roughly 6-8 points.- Cover types of cases including Start of Care, Wound Care, follow-up visits, etc.- Open to on-call rotations and potentially covering a territory mileage radius of up to 75 miles from the home clinic.- Compensation for mileage from patient home to patient home and proficiency in Pointcare EMR system required.- Local candidates accepted at the same rate.
