**Guaranteed Hours:** Not guaranteed
**Experience:** Minimum of 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty
**License:** Must have a valid RN license in hand
**Certifications:** CPR, BLS
**Must-Have:**- Proficiency in OASIS; must have used it within the last year- Proficiency in HomeCare HomeBase EMR; must have used it in the last year- Must be willing to travel up to 75 miles from home clinic- Reliable transportation and valid driver's license with required liability insurance- Open to on-call rotations and weekend assignments
**Job Description:**- Provide, coordinate, and direct home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses- Conduct home visits and attend periodic office meetings- Responsible to the Clinical Manager- Manage start of care, wound care, and follow-up visits- Work with an average of 5-8 patients per day, meeting productivity expectations of approximately 30 points per week, with different types of visits and activities contributing to point values- Be prepared to discuss weekend and on-call requirements during the interview.- Must be proficient in charting and meet agency documentation standards
