The Transitions Program works in a strengths-based, culturally relevant, and co-occurring friendly manner with participants. The program utilizes many principles of the Transition to Independence Process model that engages youth in their own recovery process towards healthier expressions of their lives. Service delivery is a collaborative process, helping participants to co-create their short- and long-term goals. We provide community-based services, meeting participants in environments where they feel most comfortable. Services are designed to meet TAY where they are, using modalities and interventions that TAY can relate to, including engaging youth in a nonjudgmental, conversational style of communication, and discussing topics that are relevant to TAY. Services place much emphasis on building a strong therapeutic alliance with our participants in order to foster the most effective outcomes.

What You Will DoThe Psychiatric Nurse Practitioner works closely with multidisciplinary clinic, community-based, or school-based treatment teams supporting children, youth and young adults with mental illness to function optimally in residential, home, community and school settings. The Psychiatric Nurse Practitioner provides medication support services and diagnostic consultation, collaborates closely with other team members, prescribes psychotropic medication, and documents all services provided. Along with the rest of the team, the Psychiatric Nurse Practitioner is active in supporting and implementing individualized, strengths-based, culturally responsive, participant/family-centered approaches to treatment.

Salary Range In accordance with California law, the expected salary range for this position is between $64.42 and $82.22 per hour. The actual compensation will be determined based on experience and other factors permitted by law.

What You Bring to The Table (requirements): RequiredActive, unrestricted CA RN license in good standing or an active, unrestricted CA NP Certificate (or Certificate Eligible) or a Master of Science or Master of Nursing degree from an accredited Nurse Practitioner program.Successful completion of a graduate-level Psychiatric NP Specialty Program Accredited by the CA BRN (preferred); OR substantive clinical training. If graduated from a non-PNP program, individual will be supervised in their practice until deemed competent to function independently.Valid CA BRN issued Furnishing License (or able to obtain within 9 months of hire).Valid DEA Schedule II certificate (or DEA Number eligible) (or able to obtain within 9 months of hire).National Board Certification in nursing required (or Board Eligible with testing for certification within 6 - 12 months of licensure or hiring, whichever is later).One or more years of experience in the specialty field of psychiatric nursing.Demonstrated cultural responsiveness in working with diverse families and communities.8. Demonstrated understanding of the developmental milestones of children, youth, or young adults to provide to provide care, treatment and services.
9. Demonstrated ability to utilize de-escalation techniques that align with the trauma informed care model as part of intervention strategy for working with participants in crisis who are responding to triggers/stressors and/or demonstrating early indicators of distress.
Clean driving record; current CA Driver's License or ability to obtain within 10 days after hire.Basic computer and keyboarding competence with the ability to use Microsoft Office Suite.Access to dependable transportation available and personal car insurance.Preferred
Certification in Advanced Practice Psychiatric Nursing.2. Experience with team-based models for assisting young adults with significant mental illness to live in the community.
3. Knowledge in philosophies of community psychiatry, evidence-based practices (particularly ACT and Motivational Interviewing), recovery, resilience, family systems, and interdisciplinary team work, etc.
4. Knowledge of theoretical and pharmacologic issues related to clinical treatment of common psychiatric disorders.
5. Experience working with young adults utilizing strength-based therapeutic approaches.
Fluency in prevailing language(s) of the community.
