Family Support Coordinators assist families who have sons and daughters eligible for DDS services (Department of Developmental Services). This person will provide home-based services to families to assist in identifying their needs in a way as to translate their needs into services and community supports.Responsibilities and Duties for Department of Developmental Services (DDS):

Inform the family and assists them in accessing supports and services that exist within the community.Assists families in obtaining all state and federal entitlement benefits such as SSI, CommonHealth clinical/medical supports, and behavioral supportsHave an understanding of the Massachusetts Special Education LawsAssists families in working with collateralsCollaborate with other agenciesSupervise support staff working in families homesFor Department of Children and Families (DCF):

Attend all Intake and Review Meetings at DCFDevelop and Implement Individualized Family Treatment PlansDocument progress and prepare reports for DCF review meetingsComplete discharge summaries for all families and all necessary documents on I Family NetIdentify services and teach skills to aid parents in keeping, or returning their children homeHave knowledge of child developmentSensitivity to poverty and cultural issuesQualifications

Bachelor's Degree in Social Work, Education, Human Services or a related field is required. Master's Degree position is also available.Up to two years of family support case management experience preferredA passion and dedication to supporting our people is a mustValid Driver’s license, reliable transportation, and acceptable driving recordMust pass company background and reference checksCheck out our website:
