Full-time, Monday - Friday (8am-4pm)

The Day Program Case Manager is an integral role in our Day Program Services that will work to build and foster professional relationships with Individuals with developmental disabilities served on his or her caseload and members of each Individuals interdisciplinary team to ensure ongoing coordination of services.

In conjunction with other departments, Individuals, families, residences, and funding sources, ensure that services meet Individual needs and promote individual choicesCoordinating or completing assessments/evaluations to determine support levels and support planning. Conducting re-evaluations on an as needed basisMonitor follow-through and effectiveness of support plans with department staff and Interdisciplinary team in cooperation of the department manager. Provide guidance to department staff in competing required documentationPlanning for and participating in planning meetings (team meetings, DHSP, ISP) actively advocating for Individuals’ needs, and contributing to overall goals of the department and individuals supportedProvide personal care and assistance to individuals consistent with individual needs as they strive to reach their goals. Providing coverage for groups as needed.Qualifications

High School diploma required (BA degree in Human Services or related field preferred)At least 1 year of experience supporting adults with developmental disabilities is preferredA passion and dedication to supporting our people is a mustValid Driver’s license, reliable transportation, and acceptable driving recordMust pass company background and reference checksCheck out our website:
