The Position:
The Nurse Practitioner provides primary patient care for specifically assigned patients to include assessments, care planning, evaluation and education of patients receiving infusion therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site setting.
Responsibilities Include:Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patientsOrders and interprets diagnostic and therapeutic tests relative to patients age-specific needsPrescribes appropriate pharmacologic and non-pharmacologic treatment modalities upon consultation with the patients physicianMonitors the effectiveness of treatment and interventionsFacilitates staff, patient and family decision making by providing educational toolsDevelop, in conjunction with other clinical team members, each patients Care Map and evaluate patients response to therapy during each home visit or encounterAppropriately discharge primary patients and complete discharge summary within 7 days of dischargeDevelop and maintain caseload scheduling and communications on a weekly and monthly basis in updating schedule with changesDocument all patient care activities and interventions as required by organizational policy and procedure and law and regulationCoordinate care via live telephone conversations and/or voicemail with the team as determined by the urgency of information to be relayed
Schedule:Part time 24-32 hours per week
Requirements
Graduate of an accredited school of nursingGraduate of an accredited Nurse Practitioner ProgramCurrent Advanced Practice Registered Nurse (APRN) LicensureCurrent provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS)Excellent IV skillsStrong computer skills, including Word, Excel, and PowerPointMinimum 3 years nursing experience with background in infusion therapyExcellent customer service skills
