Physical Therapist, PT Job Description:
The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.
Physical Therapist, PT Responsibilities:
Assessment and Evaluation: Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait.
Individualized Care Plans: Develop individualized treatment plans based on assessments, with clear goals for improving residents' mobility and reducing pain.
Section GG Data Collection: Accurately and consistently collect Section GG data as required for Annual, Quarterly, Falls, and Significant Change assessments.
Treatment: Administer physical therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.
Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
Documentation: Comfortable and knowledgeable with Point of Service (POS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
Interdisciplinary Collaboration: Collaborate with the rehab team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.
