Responsabilities:
Assist the Physical Therapist in carrying out treatment plan and gather objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change screenings.
Treatment Support: Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.
Monitoring: Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.
Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( Net Health/PCC).
Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.
Team Collaboration: Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other rehab staff.
