JOB DESCRIPTION :
The Unified Government Public Health Department is seeking a Family Support Specialist (Program Coordinator) for its Healthy Families Wyandotte (HFW) Program. HFW is an evidenced-based, intensive home visitation program. The Family Support Specialist is responsible for initiating and maintaining regular and long-term (up to 3 years) contact/support with families. This activity will occur primarily within the family's home; each visit should last one hour. The interventions should be family-centered, strength-based, and directed at establishing a trusting relationship; assisting in strengthening the parent-child relationship; assisting parents in improving their skills to optimize the home environment; improving the family support system; and increasing the family's ability to problem solve and assume the role of advocate for themselves and their children. The activities may also include identifying and referring for contact/appointments at other supportive agencies, including health care appointments. The Family Support Specialist will also be responsible for assisting the family in establishing goals and a plan for the accomplishment of those goals, as well as the assessment of the normal growth and development of the target child. Must be able to work with a diverse client population, work in a team setting, maintain an appropriate caseload, complete documentation on time, exercise good judgment, and refer clients to community resources as appropriate. Bi-lingual in Spanish/English or Burmese/English is a plus.
MINIMUM QUALIFICATIONS :
Bachelor's degree in social work, nursing, special or early childhood education, child development or a related field, experience in working with or providing services to children and their families or experience; or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job. A valid Kansas driver's license and reliable transportation are necessary.
Salary Range:
$25.96-$34.72/hr.
